To get started I have attaching two new medical questionnaires in this email, please fill out these questionnaires and send them back to me via email or fax. Also fax me your most recent medical records, that show you’ve been treated and suffer from chronic pain. If you have MRI’s, or X-Rays please send those as well. When I receive everything from you, I will send it to the doctor for pre-approval and we can go from there.

Also please answer these few questions so I can put it in your folder.

1. Name of Primary Physician
2. Phone # of Primary Physician
3. Last date of visit with them.

That is just for our records and as for your medical records, everyone has to be approved by the doctor for your visit, and they are requiring updated Medical records. If you could go ahead and fax those to me at (352) 397-2802, I can get you ready to see the doctor.

Good news for those who have still not been able to see a doctor. In addition to our current doctor list, we have entered into a contract that will give us a least one doctor in all 48 states on the main continental US. The first 3 states on board and ready to go are Seattle Washington, Dayton Ohio, Chicago Illinois, Oregon, Atlanta GA, Des Moines IA, Shelton CT, Salt Lake City UT, Birmingham AL, Minneapolis MI, Louisville & Elizabethtown KY, Milwaukee WI, Boston & Springfield MA, Richmond VA.. Our ultimate goal is to have most of the other states scheduling and open up nationwide within 14 to 28 days. The others will be available as we get all their information, exact locations, and schedules in place.

We have also added several new services. Most of our doctors can also see you for Diabetes, Hypertension, and E.D.

I have a question that I think is fairly decent. I obviously do not have medical insurance and am a statistic of the unemployment line. I was in a car wreck in May 2008 and have had problems ever since. I also have been diagnosed with an anxiety disorder. The medical records department of the hospital I was brought to by ambulance wants to charge me $40.00 for the first 10 pages and over a buck a page after that if I get my own records. However, they will send them for free to another doctor. Is that possibility? I know that there are consent forms I will have to sign, which I will happily to just to be seen by someone who will supply me with adequate medication to help take the edge of my pain. Please let me know.

Very high consultation, and it doesnt always work out, be careful, havent heard anything positive or negative from this place.

3.You will have the choice of either Direct Script $60 (this is fill filled in your pharmacy) or Pharmacy Fill $95(this is fill filled in our pharmacy) and that will be charged separately, but at the same time as the consultation fee.
4.Then you will get one month supply, and towards the end of the month we contact you to set up your next consultations.
5.Your next 11 consultations will be charged $125 monthly consult fee, plus the Direct script fee or Pharmacy fill, which ever you chose initially.
6.And that will repeat monthly.

*Please note* - So doctors do require monthly face to face consultations, until they feel that you can switch to monthly telephone consult. At the doctor’s discretion, they will decide when that happens . The price will still be $125 monthly even if it’s a telephone consult.

I called these guys today, after sending two emails, and receiving no response, until after I called today. I'm sorry to say the customer service rep seemed very disinterested in even talking to me, she didn't even know what she was talking about it sounded like. I asked if there was a doctor in the Minneapolis area, she slowly said "I'll check." followed by 2 minutes of silence, after which I said "St. Paul maybe?" and then she answered.

I had questions about the service, I want to know if I'm a good candidate for it, otherwise why even bother with it - I'm not going to spend $260 on the consultation, plus $60 script fee, plus one other fee - for $90 something.

I was able to gain the knowledge that I only have to see the doc annually in my area, those are $170.

Anyway, I informed the women on the phone that I only have an X-ray of my shoulder, and my records would consist of that and only urgent care visits for pain because I'm uninsured. I asked if that would be okay and she didn't really know. Can I get an answer on that? I'd like to be able to make my appt by 5pm today to qualify for the DISCOUNT. Can anyone help?

Chris, if you give us some more info maybe we can be of more help. Why did your doctor stop prescribing? It can't just be because of the PA because you can always go around her when you make an appointment and make sure you see him. Did he decide your dosage was too high? Did you call in for early refills often? Run out of medicine too soon? Have pill counts that were wrong? Go to another doctor and got a narcotic? (like going to a dentist and getting vicodin for a procedure? PM clinics hate that).

You don't have to spill your guts but if we have a little more info than "my PCP quit prescribing" it might be helpful. And was it a PCP or a PM doc? Because if it was a PCP I can see why he would stop. Too much heat on a regular doctor. A PM doctor doesn't get too upset with the DEA going over their records because they are careful to keep good records. Just some thoughts.......
